Just got the rules so I have a couple of questions.

In a 200 points list, how do you points cost the commanders. For example the Nikephorian Byzantine list has a Command of +6. If I gave +2 to each commander, what would that cost in points?

Page 8 says each army has a mandatory camp and yet the list above, again for example has the camp as 0 - 1 so presumably this means it is optional.

A brilliant commander is 6pts, so three of them 18pts.

Every army has a camp, but the list allows you to upgrade it to 'fortified' (0-1).

The costs for commanders are on page 74 of the rules

To use all of your +6 you could as you say take 3 Brilliant commanders, which would then work out as 18 points out of a 200 point army. 

As the table on p74 suggests, you can get a “rebate” on these values by either making the commander unreliable or embedding them permanently into a unit.

Both of these have downsides.... hence the points reduction

Unreliable commanders downsides are set out on p73, and the downsides to embedded commanders are set out on p25.

If you are starting out I’d recommend avoiding both of these at least for a while!

Alan is right about the camp - the cost for fortifying it (which is t allowed for many lists anyway) is also on p74.

+6 would also be a strategist (if allowed by list) which counts as +2 and 2 x Brilliant.
